About the role

Hospicio Toque de Amor is seeking a full‑time Registered Nurse to provide in‑home hospice care in Manatí and surrounding areas. The RN will work directly with terminally ill patients, their families, and an interdisciplinary team to ensure compassionate, culturally sensitive end‑of‑life support.

Key responsibilities

• Assess patient conditions, monitor symptoms, and document status in electronic health records.
• Administer medications and perform nursing procedures according to established care plans.
• Educate patients and caregivers on symptom management, safety, and comfort measures.
• Coordinate with physicians, social workers, and other team members during care plan meetings.
• Communicate changes in condition and care needs to families promptly.

Required profile

• Current RN license in Puerto Rico with a BSN and eligibility to practice in hospice settings.
• Strong communication and interpersonal abilities for interaction with patients, families, and team members.
• Organizational and time‑management skills to handle home visits, documentation, and care coordination.
• Valid driver’s license, reliable transportation, and willingness to travel within the North and Metropolitan area.
