Patient Care Coordinator

About the role

Triple‑S is seeking a Patient Care Coordinator to join its telehealth team in Puerto Rico. In this position you will be the first point of contact for patients using care stations, ensuring a smooth, courteous experience in both English and Spanish while supporting high‑quality, affordable healthcare delivery.

Key responsibilities

• Answer incoming calls within 60 seconds, greet patients warmly, and verify demographic and insurance information.
• Accurately record patient data in the On‑Med Command Center Records system and upload identification documents.
• Monitor patient volume across stations, coordinate provider consultations, and manage call order to avoid delays.
• Conduct post‑visit follow‑up calls, document outcomes, and ensure pending needs are resolved.
• Arrange follow‑up appointments with primary care providers within seven days of the encounter.
• Provide patients with clear instructions on telehealth processes, wait times, and station usage.
• Report technical issues with On‑Med, Athena, and Coretek systems to support teams and document resolutions.

Required profile

• High school diploma required; associate’s degree in healthcare, administration, or related field preferred.
• At least one year of experience in patient registration, call‑center operations, customer service, or healthcare administrative support.
• Ability to communicate professionally in English and Spanish.
• Commitment to HIPAA compliance and maintaining a secure remote work environment.

Required skills

• Proficiency with the On‑Med Command Center Records system.
• Experience using Athena and Coretek platforms.
