Clinical Pharmacist – Call Center (San Juan)

About the role

Optum is seeking a Clinical Pharmacist to join its Clinical Engagement Services Center in San Juan. The pharmacist will conduct telephonic medication reviews, provide adherence counseling, and support disease‑state management for members.

Key responsibilities

• Engage members by phone, schedule and conduct pharmacist consultations for clinical programs.
• Use prescription claims data to generate clinical recommendations, addressing gaps in care, high‑risk medications, compliance, drug interactions, and therapeutic substitutions.
• Assess medication regimens for unnecessary or inappropriate drugs, doses, and adverse reactions.
• Provide patient education on medication use and disease states.
• Document care notes and recommendations accurately and concisely.
• Participate in departmental meetings to improve programs and processes.

Required profile

• Active, unrestricted Pharmacist license (or Pharmacist Intern) in Puerto Rico.
• Experience working independently within a cross‑functional team.
• Strong organizational skills and ability to manage a flexible schedule with rotating shifts.
• Solid English communication skills, both written and verbal.

Required skills

• Computer proficiency, including Microsoft Office applications.
