RN Clinical Care Manager – Kidney Management

About the role

The Registered Nurse Clinical Care Manager will build and maintain relationships with physicians, provider staff, and Healthmap members enrolled in the Kidney Management Program. The role focuses on care coordination, performance improvement, and education to enhance health outcomes for patients with chronic kidney disease.

Key responsibilities

• Act as liaison between Healthmap, provider practices, and members to ensure engagement and program performance.
• Develop new provider partnerships and manage an assigned caseload of members.
• Identify health‑outcome improvement opportunities using provider data and implement best‑practice education.
• Educate providers on CKD, ESRD, and renal replacement therapies and support HEDIS/STAR measure improvement.
• Advocate for members, connect them with community resources, and coordinate care navigation.
• Maintain documentation compliant with NCQA standards and HIPAA regulations.

Required profile

• Active, unrestricted RN license with BLS certification; ACLS certification as required.
• Associate, diploma, or bachelor’s degree in nursing.
• 3+ years progressive experience in healthcare services, clinical operations, quality, or care management.
• Bilingual fluency in English and Spanish and residency on the island.
• Valid driver’s license, automobile insurance, and willingness to travel within the region.
